1. Eat a variety of foods

For good health, we need more than 40 different nutrients, and no single food can supply them all. It is not about a single meal, it is about a balanced food choice over time that will make a difference!

  • A high-fat lunch could be followed by a low-fat dinner.
  • After a large meat portion at dinner, perhaps fish should be the next day’s choice?

2. Base your diet on plenty of foods rich in carbohydrates

About half the calories in our diet should come from foods rich in carbohydrates, such as cereals, rice, pasta, potatoes, and bread. It is a good idea to include at least one of these at every meal. Wholegrain foods, like wholegrain bread, pasta, and cereals, will increase our fibre intake.

3. Replace saturated with unsaturated fat

Fats are important for good health and proper functioning of the body. However, too much of it can negatively affect our weight and cardiovascular health. Different kinds of fats have different health effects, and some of these tips could help us keep the balance right:

  • We should limit the consumption of total and saturated fats (often coming from foods of animal origin), and completely avoid trans fats; reading the labels helps to identify the sources.
  • Eating fish 2-3 times a week, with at least one serving of oily fish, will contribute to our right intake of unsaturated fats.
  • When cooking, we should boil, steam or bake, rather than frying, remove the fatty part of meat, use vegetable oils.

4. Enjoy plenty of fruits and vegetables

Fruits and vegetables are among the most important foods for giving us enough vitaminsminerals and fibre. We should try to eat at least 5 servings a day. For example, a glass of fresh fruit juice at breakfast, perhaps an apple and a piece of watermelon as snacks, and a good portion of different vegetables at each meal.

5. Reduce salt and sugar intake

A high salt intake can result in high blood pressure, and increase the risk of cardiovascular disease. There are different ways to reduce salt in the diet:

  • When shopping, we could choose products with lower sodium content.
  • When cooking, salt can be substituted with spices, increasing the variety of flavours and tastes.
  • When eating, it helps not to have salt at the table, or at least not to add salt before tasting.

Sugar provides sweetness and an attractive taste, but sugary foods and drinks are rich in energy, and are best enjoyed in moderation, as an occasional treat. We could use fruits instead, even to sweeten our foods and drinks.

6. Eat regularly, control the portion size

Eating a variety of foods, regularly, and in the right amounts is the best formula for a healthy diet.

Skipping meals, especially breakfast, can lead to out-of-control hunger, often resulting in helpless overeating. Snacking between meals can help control hunger, but snacking should not replace proper meals. For snacks, we could choose yoghurt, a handful of fresh or dried fruits or vegetables (like carrot sticks), unsalted nuts, or perhaps some bread with cheese.

Paying attention to portion size will help us not to consume too much calories, and will allow us to eat all the foods we enjoy, without having to eliminate any.

  • Cooking the right amount makes it easier to not overeat.
  • Some reasonable serving sizes are: 100 g of meat; one medium piece of fruit; half a cup of raw pasta.
  • Using smaller plates helps with smaller servings.
  • Packaged foods, with calorie values on the pack, could aid portion control.
  • If eating out, we could share a portion with a friend.

7. Drink plenty of fluids

Adults need to drink at least 1.5 litres of fluid a day! Or more if it’s very hot or they are physically active. Water is the best source, of course, and we can use tap or mineral water, sparkling or non-sparkling, plain or flavoured. Fruit juices, tea, soft drinks, milk and other drinks, can all be okay – from time to time.

8. Maintain a healthy body weight

The right weight for each us depends on factors like our gender, height, age, and genes. Being affected by obesity and overweight increases the risks of a wide range of diseases, including diabetes, heart diseases, and cancer.

Excess body fat comes from eating more than we need. The extra calories can come from any caloric nutrient – protein, fat, carbohydrate, or alcohol, but fat is the most concentrated source of energy. Physical activity helps us spend the energy, and makes us feel good. The message is reasonably simple: if we are gaining weight, we need to eat less and be more active!

9. Get on the move, make it a habit!

Physical activity is important for people of all weight ranges and health conditions. It helps us burn off the extra calories, it is good for the heart and circulatory system, it maintains or increases our muscle mass, it helps us focus, and improves overall health well-being. We don’t have to be top athletes to get on the move! 150 minutes per week of moderate physical activity is advised, and it can easily become part of our daily routine. We all could:

  • use the stairs instead of the elevator,
  • go for a walk during lunch breaks (and stretch in our offices in between)
  • make time for a family weekend activity

10. Start now! And keep changing gradually.

Gradual changes in our lifestyle are easier to maintain than major changes introduced all at once. For three days, we could write down the foods and drinks we consume throughout the day, and make a note of the amount of movement we made. It won’t be difficult to spot where we could improve:

  • Skipping breakfast? A small bowl of muesli, a piece of bread or fruit, could help slowly introduce it into our routine
  • Too few fruits and vegetables? To start with, we can introduce one extra piece a day.
  • Favourite foods high in fat? Eliminating them abruptly could fire back, and make us return to the old habits. We can choose low fat options instead, eat them less frequently, and in smaller portions.
  • Too little activity? Using the stairs daily could be a great first move.

That’s a natural metabolic state where your body breaks down fat and converts it to ketones to use as energy.

Normally, your body would just use the glucose in carbs for energy, but if you aren’t eating many carbs, it uses your body’s stored fat instead. This is how the keto diet leads to weight loss.

Your body doesn’t get into this state of ketosis immediately. It takes about three to four days for your body to run out of its normal fuel, after which you’ll enter ketosis and start burning fat as energy for daily activities.

To get your body into ketosis, it’s typically recommended to eat less than 50 grams of carbohydrates per day.

This means cutting out high-carb foods like breads, pastas, potatoes, sugary foods and the like. It also means no alcohol—sorry!

In other words… it doesn’t take a ton of food to reach that carb limit.

Another potential drawback of the keto diet is that it’s common to develop the “keto flu” when first starting the diet. This is totally normal as your body adjusts to no longer having the amount of carbs it’s used to, but that doesn’t mean it’s fun to deal with. No one likes being sick!

  • nausea or vomiting

  • being unable to exercise

And, to that last point: the keto diet is not the best choice for athletes or those looking to add muscle. To put on muscle or perform strenuous workouts, you’ll generally need a more protein-heavy nutrition plan.

Recruiters report that behavioral interviewing and questions that evaluate emotional intelligence are becoming a larger part of the physician interview process.

In essence, interviewers want to know: “Why should we hire you, and what will you bring to our team or practice?”

And they have a right to be concerned.

Patient outcomes, practice reputation and team dynamics are hanging in the balance. Health care employers need to be extremely careful that they “hire well.”

Yet physician candidates need to be just as careful as they evaluate each job opportunity.

What you should ask potential employers or partners

Employers also want to know that you are invested in the interview process and expect to hear a few questions from you. And you’ll want to be sure that this specific job is a good fit at this point in your career.

As the physician job candidate, you should prepare some thoughtful questions to ask the interviewer(s)–and anyone else you meet at the organization who may contribute to the hiring process, or is part of the clinical team.

So, do your homework ahead of time. Get some basic information about the organization, including size and location, the services it offers, the size and composition of the staff, etc.

What is your decision-making process?

Who makes the decisions, and are there opportunities for you to be involved in that process? Along those lines, you might also inquire about the relationship between the administration and the physician staff.

What are the biggest challenges currently facing the organization?

What do you anticipate will be the biggest challenges in the next few years? Every health care organization has some challenges. Their answers to these types of questions can help you get an idea of the environment you may be entering, and how the group addresses problems.

What is my role and how does it fit in the organization?

Ask more about this physician job, and the role of your specialty, in the organization. How does this particular job fit into the organization’s current plans and its plans for the future? What will be expected of you? How will you be reviewed or assessed?

What kinds of opportunities are available for physicians?

Ask about what kind of professional development and leadership opportunities are available for physicians. Will you be required to serve on committees, lead any initiatives, or mentor other physicians? If so, how would that work? If it is a group practice, is there partnership potential?

What would be included in my compensation?

Many experts suggest waiting until the interviewer brings up this topic, but you should be prepared just in case. Find out as much as you can ahead of time from your recruiter about salary, benefits, opportunities for bonuses, profit-sharing (if applicable), and student loan repayments. If you are offered the position, that is generally the best time to negotiate.

How do you support physician wellness amid daily demands?

Ask questions about patient loads, support positions, work scheduling and on-call duties, if you haven’t already tracked down this information prior to your interview. If you have the opportunity to meet with any of the organization’s physicians, you might ask their experiences and how they approach the issue of work-life balance.

If you’re interviewing with a group practice, you should also prepare some questions that specifically deal with the way the practice functions, including their structure and management issues. Are you ready to retire from your career as a physician, or is it on the horizon? Before you hang up your white coat for good, there are some important considerations that deserve attention.

While more than two of five physicians are projected to be on the cusp of retirement in the next 10 years according to the Association of American Medical Colleges (AAMC), the added strain of COVID-19 has some physicians considering an early retirement. Whether you’ve expedited your physician retirement timeline or not, let’s review the six things you should do to prepare for a smooth transition.

1. Is Burnout Driving Your Physician Retirement Timeline?

If so, you’re not alone. A recent Medscape survey uncovered that 42% of physicians report being burned out. Many of these doctors whose long-term stress has led to emotional and physical exhaustion, lack of professional fulfillment, and dwindling feelings of accomplishment are seriously considering leaving medicine altogether. Of those who responded to the survey, 55% said they were overwhelmed with too many bureaucratic tasks and working excessively long hours. If burnout is behind your decision to retire now versus later, consider approaching your healthcare administration about working fewer hours. Jackson Physician Search conducted a survey on retirement and found that nearly 28% of physicians planned to work part-time or elsewhere upon retiring from their current position. Your current employer may entertain other options including a part-time schedule or taking on telemedicine hours.

2. What Will You Do with All Your Newfound Free Time?

The pandemic has many physicians working the volume of hours that resemble the old days of residency. But even prior to the pandemic, doctors worked an average of 50 or more hours per week. When you’re used to that kind of pace, suddenly having all of that time at your disposal can be a bit unsettling. Make a plan for developing new hobbies and other interests, network with physicians who are also making the transition, or consider ways in which you would enjoy giving back to the medical field.

3. Is Your Financial House in Order?

Prior to 2020, you were probably ecstatic with your retirement account’s financial performance and the unprecedented market gains we were experiencing. Now, like many of us, those financial gains likely took a noticeable hit. Many physicians subscribe to a financial plan called FIRE or Financial Independence, Retire Early. Working toward FIRE means that you have a plan to leverage your high demand, high salary career toward the ability to retire when you want. Even if you haven’t subscribed to the principles of FIRE, it is important to discuss your options with a financial advisor.

4. Will You Maintain Your License to Practice Medicine?

Even if you plan to fully retire, you should still consider maintaining your medical license. While it varies by state, allowing your license to lapse can take up to six months or more to renew if the need arises. During the early days of the pandemic, some physicians felt called to come out of retirement to help. Since the future can be so wildly unpredictable, many physicians plan to keep their license and board certifications active for up to five years.

5. Have You Reviewed the Legal Considerations of Retirement?

Unlike some occupations where a person retires after a long productive career and can generally walk away unencumbered, that’s often not the case for physicians. Here are a few that you should address early:

Employment Contracts. In most cases, you will have an employment contract that contains stipulations about leaving the hospital or practice. Review the agreements or consult your lawyer to ensure you understand all aspects of your pending retirement.

Patient Records. Depending on your particular practice setting, how you handle patient records will vary. If you own your own practice and plan to shut it down entirely, you should consult your state medical board regarding records retention and accessibility of patient information. In other settings, the records question can usually be handled by your employer.

Notifications. Multiple notifications need to occur, including your patients, state licensing board, professional associations, and employer. We will cover employer notifications in more detail below, but the others are also worthy of consideration.

  • Patient notification – While it is likely that your state will have specific requirements, providing your patients with at least a 60-day notification is often standard. Where practical, and with higher-risk patients, notification by certified mail with a return receipt is the safest course of action.
  • State licensing board – Before retirement, it is vital that physicians contact the state licensing board, state medical society, and the American Medical Association. Depending on the state, you may be able to maintain an active license with certain restrictions at a reduced cost.
  • Professional associations – Over the years, you have likely participated in various professional associations, specialty boards, and other types of medical societies. Each organization has different policies, but it is a good practice to reach out to them with notification.

6. When Will You Notify Your Employer?

This can be a tricky area for physicians who want to provide plenty of notice but are unsure how much is necessary. In the aforementioned Jackson Physician Search retirement survey, 80% of physician respondents stated that it was their responsibility to initiate the retirement discussion with their employer. The caveat is that only 52% of them feel comfortable discussing the subject with their administrators. On the other side, 37% of administrators thought it was their responsibility to initiate retirement discussions, but 74% said they were very comfortable having that conversation. Depending on your specialty, it can take up to a year to fill a physician vacancy. That may shed some light on why 50% of administrators responded that the ideal notification timeframe is one to three years, while 40% of physicians stated that six months or less was sufficient.

The stuff to Run a Great Virtual Meeting

As companies scramble to protect employees from the spreading coronavirus with travel restrictions and remote work arrangements, there’s a distinct possibility that in-person meetings with teams, customers, or suppliers may be canceled for days — or potentially weeks.

Under the best of circumstances, as soon as one or two attendees “dial in” to any meeting, productivity starts to suffer.  There’s a long list of reasons. Attendees often interpret virtual meetings as a license to multi-task. Meeting organizers tend to be less careful with the purpose and design of the conversation. And it’s not uncommon for one or two attendees to dominate the discussion while others sit back and “tune out.”

But it doesn’t have to be this way. Virtual meetings — even impromptu ones sparked by fears of a contagion — can be run more effectively, using basic meeting best practices and easy-to-use, inexpensive technology.

Here are 12 steps you can take to make that happen:

1. Use video. To make people feel like they’re all at the “same” meeting, use video conferencing rather than traditional conference dial-ins. Technology — such as Zoom, Skype, and GoToMeeting — helps to personalize the conversation and to keep participants engaged.

2. That said, always provide an audio dial-in option. Video conferencing can work very well, but it relies on a strong internet connection that may not always be available. People need the ability to participate via audio, but make it clear that video-first is the new norm.

3. Test the technology ahead of time. Nothing kills momentum at the start of a meeting like a 15-minute delay because people need to download software, can’t get the video to work, etc. Prior to a virtual meeting, all participants should test the technology and make sure they are comfortable with the major features.  And remember, supplier or customer conversations may require your team to familiarize themselves with different software packages.

4. Make sure faces are visible. Video conferences are more effective when people can see each other’s facial expressions and body language. Ask individuals to sit close to their webcam to help to recreate the intimacy of an in-person meeting.

5. Stick to meeting basics. Prior to the conversation, set clear objectives, and send a pre-read if appropriate. During the session, use an agenda, set meeting ground rules, take breaks, and clearly outline next steps (including timing and accountabilities) after each section and at the end of the meeting.

6. Minimize presentation length. The only thing worse than a long presentation in person is a long presentation during a virtual meeting.  Meetings should be discussions. Background information should be provided beforehand. If someone needs to present, use screen sharing to guide the conversation, so attendees can literally “be on the same page.” But prioritize conversation to maximize the time people are looking at each other.

7. Use an icebreaker. Although we’re not big fans of them, it’s important to use every tool to reinforce interpersonal relationships when people may be feeling isolated.  Also, it’s important to know if a participant may have a close friend or relative fighting the virus, so some type of “check in” is in order.

8. Assign a facilitator. It’s usually harder to manage a virtual discussion than an in-person one. It can be helpful to assign one individual to guide the conversation, allowing the other participants to focus on the content. The facilitator can also use a polling system to “take the pulse” of the group on certain questions and ensure that all voices are heard.  The facilitator should also be able to resolve basic questions on the technology being used.

9. Call on people. Getting everyone to participate without talking over each other is one of the more challenging aspects of running a virtual meeting. To forestall this, we recommend periodically calling on individuals to speak, even by virtually “going around the table” before a decision is finalized. Some software packages even allow attendees to “raise a hand” if they want to. This can help the facilitator drive closure without the risk of excluding an introverted participant’s views.

10. Capture real-time feedback. Gathering and processing high-quality input during a virtual meeting can be challenging, especially since visual cues are harder to read.  Use a phone-based survey tool like Poll Everywhere to collect on-demand feedback from attendees on specific topics in real time. Keep the polling open, separate from the videoconference to avoid disrupting the conversation. Participants will need clear instructions on how to use the system and practices, but groups get the hang of it very quickly and it’s well worth the effort.

11. Don’t be afraid to tackle tough issues. Meeting virtually is a learned behavior, and you’ll be amazed how much you can get out of it once you and your team begin to be comfortable working this way. It may seem natural to wait to discuss tough issues until everyone is in person, but that may not be an option. So don’t shy away from controversial topics.

12. Practice once or twice while you’re still together. Hold your next staff meeting virtually, with each executive sitting in their office and hooking into the meeting with no assistance. After the meeting concludes, gather and debrief about the experience.  What went well, and what didn’t?  How can you evolve your virtual meetings to make them as productive as when you meet in person?

Not being able to work together in the same room with colleagues may become a major challenge in the next few weeks. To make virtual meetings work, you might need to adjust how your team conducts them.  But a small investment in preparedness now could have a huge impact if that time comes.
